For a confined space event in utilities, the enquiry should also preserve the physical scene, electronic records, appointment chain, risk assessments, permits, inspections, maintenance history, competence evidence and the instructions actually communicated to the people exposed.

Rescue without creating a second incident. Isolate residual energy. Account for people.
Control access. Photograph condition. Secure documents, data, CCTV and perishable evidence.
